Sound Request Form

This form is for RSOs making Market Wednesday requests ONLY.

Amplified sound hour reservations for Market Wednesdays are open to all RSOs pending completion of this form, necessary contracts, and approval. Organizations can only request the amplified sound hour once per semester, and dates are given on a first-come, first-serve basis. The DJ/band/performing act is to be set up on the steps in front of Moore Auditorium in the Union Courtyard only. Refer to http://union.fsu.edu/sac/market for all policies regarding Market Wednesdays.

Please complete the information below first to request the space and date. After confirmation of the date, you may need to complete additional contracts. If hiring a DJ, you must complete a DJ Contract and a Facilities Use Agreement at least 10 business days prior to the event and turn these in to Marsha Hadder in the Student Activities Center. These contracts must be completed even if no payment is being offered. Contracts can be found at http://union.fsu.edu/sac/market or http://union.fsu.edu/sac/eventplanning
